Britain's Supreme Court has declined to hear an appeal from a mother and father who want to take their terminally ill toddler to the Vatican's Bambino Gesù Children's Hospital for treatment.
Tom Evans, 21, and Kate James, 20, have made another application to the Supreme Court after losing a second fight over Alfie Evans in the Court of Appeal.
A Supreme Court spokeswoman said: "Having considered submissions from the parties 'on paper", in the usual way, the Supreme Court of the United Kingdom has refused permission for the parents to appeal'.
A lawyer representing the couple on Monday asked Court of Appeal judges to rule that Alfie should be allowed to travel to a foreign hospital.
Three appeal court judges endorsed a plan drawn up by doctors earlier this week.
The Supreme Court acknowledged that this was a "desperately sad case...principally of course for Alfie's parents for they love their little boy dearly and want to do all in their power to keep him alive".
Alfie Evans, born May 9, 2016, is lying critically ill in Alder Hey Children's NHS Foundation in Liverpool. That has inevitably prolonged the period over which he has been given treatment that was determined in February not to be in his best interests.
The justices said: "Alfie looks like a normal baby, but the unanimous opinion of the doctors who have examined him and the scans of his brain is that nearly all of his brain has been destroyed".

The Supreme Court had also dismissed the suggestion that Alfie was either "detained" or "unlawfully detained".
"We are appealing today because we have got to act quickly", Chief executive Andrea Williams said.
